Did you miss the wellness fair?

Don't worry - you can schedule your biometric screening appointment today

It's time again to complete your annual online health assessment. All employee subscribers to a city health plan are required to complete an annual health assessment or pay a $25-monthly surcharge begining in November.

What you need to complete the assessment:

• Blood pressure

• Total and HDL cholesterol

• Height and weight

• Waist circumference

Two ways to get your biometric measurements:

• Sign up for a free biometric screening appointment at www.registerformyevent.com/coh or call 800-694-4982

• Make an appointment with your doctor for your free annual physical

Appointments start on August 15 and run through September 25. Schedule your appointment online at www.registerformyevent.com/coh, appointments are listed by department, but you are not limited to any location. Or, you can call 800-694-4982. There are a limited number of screening events this year, so be sure to log on and make your appointment today.

Important health assessment reminders:

• Go to mycigna.com to access the health assessment

• Use actual biometric measurements that were obtained no earlier than June 1, 2012

• Do not choose vague answers like "I'm not sure"

• Do not use the drop down box for cholesterol

• Be sure to go all the way to the end of the assessment
