Screenings begin this week.
Have you scheduled your appointment yet?
Appointments for biometric screenings are filling up fast! Schedule your appointment online at www.registerformyevent.com/coh, appointments are listed by department, but you are not limited to any location. Or, you can call 800-694-4982. There are a limited number of screening events this year, so be sure to log on and make your appointment today.
Note: It is not necessary to fast for the screening and you should take your daily medication(s) as directed.
All employee subscribers to a city health plan are required to complete an annual health assessment or pay a $25 monthly surcharge begining in November. Registering for a biometric screening is a quick and easy way to get the numbers you need to complete the assessment.
If you attended the George R. Brown Wellness Fair and did not receive or lost your biometric screening results you can call Cigna at 800-694-4982.
What you need to complete the assessment:
• Blood pressure
• Total and HDL cholesterol
• Height and weight
• Waist circumference
Can't make it to a screening?
Make an appointment with your doctor for your annual physical. If you haven't had one this year, your appointment will be at no charge. Just be sure to let your doctor know what biometric measurements you need to complete the assessment.
Important health assessment reminders:
• Go to mycigna.com to access the health assessment
• Use actual biometric measurements that were obtained no earlier than June 1, 2012
• Do not choose vague answers like "I'm not sure"
• Do not use the drop down box for cholesterol
• Be sure to go all the way to the end of the assessment
