The City of Houston 2013 Benefits Open Enrollment began Sunday, March 17, 2013 and continues through Monday, April 15, 2013.

During Open Enrollment You May

• Enroll in plans

o Medical

o Dental

o Vision (NEW!!)

• Change plans

• Add or drop dependents to/from these plans

• Waive coverage (must waive again)

• Enroll/Re-enroll in the Healthcare Flexible Spending Account

• Enroll/Re-enroll in Supplemental Insurance

• Elect/waive the voluntary disease prevention discount (smokers vs. nonsmokers)

• Enroll/Increase/Decrease/Waive Voluntary Life Insurance and/or update beneficiaries

Healthcare Flexible Spending Account

• Maximum yearly contribution remains $2,500.00. Minimum yearly contribution remains $240.00. All contributions are pre-taxed.

• Eligible expenses incurred by April 30th can be claimed until July 29th.

• Use the AFLAC NOW Card for instant Reimbursements on eligible expenses.

• Participants must enroll or re-enroll using Choicelinx by Sunday, April 14, 2013 for changes to be effective May 01, 2013.
